Support Worker Positions

Key Duties and Responsibilities:

  • Assists case management in identification of potential problems and reports any difficulties.
  • Provides input to the team with regard to the development of appropriate program plans to achieve Personalized Service Plan (PSP) objectives.
  • Contributes to the evaluation of individuals’ progress and makes either written and/or verbal reports to the team.
  • Provides emotional support and feedback to individuals and their families
  • Acts as a positive role model.
  • Provides life-skills support and behaviour management
  • May assist with hygiene, grooming, dispensing and taking of medications
  • May provide transportation and assist persons served in attending appointments
  • Maintains necessary reports and data entry on individuals and ensures that all required documentation is accurate and complete.
  • Identifies social, economic, recreational and educational services in the community that will meet individuals’ needs.
  • Monitors the operation of the residence including quality control concerning cleaning equipment, indoor and outdoor maintenance, inventory and food services.

List of Requirements:

Upon commencement of orientation, all support staff must have and retain the following in their personnel file:

  • Resume
  • Completed Interview Questions
  • Current clear Criminal Record Checks: RCMP & CCRC
  • MCFD Screening
  • New & Young Workers Orientation Checklist
  • Physician’s Declaration
  • Current Emergency First Aid – Community Care
  • Immunization Records including clear current TB test/ Tetanus verification
  • Copies of all educational and professional documentation
  • Three written reference letters
  • Copy of valid BC Class 4 or 5 Driver’s License
  • Supporting Individuals Through Valued Attachments (SIVA) Training Certificate – Training will be provided by Applegate Residential Programs Ltd without cost to new employees.
  • Driver’s License Abstract (Updated Yearly)
  • Signed Employment Agreement (Updated Yearly)
  • Employee Emergency Medical Information (Updated Yearly)
  • Completed TD1/TD1BC/Direct Deposit Forms (Updated Yearly)
  • Completed Staff Orientation Manual/ Annual Policy Review (Updated Yearly)
  • Other forms may be required based on the job description
